Advocacy for Children's Health and Education Services
The A.C.H.E.S. Program Provides vouchers for medical, dental, and mental health care services to uninsured children who qualify financially. Vouchers are available for acute and Well-Child exams/preventative care.

Patient Navigator Team
This program assists Grand County residents with barriers to healthcare. Our navigators are registered nurses who can assist in care coordination, disease education, access to care, and more. We work one-on-one with patients and their doctors to empower patients to better manage their own care.

Behavioral Health Navigators
Contact if you need help understanding the confusing mental health system, need resources to get the mental health care you need when you need it, or need to talk to a trained therapist but are not sure of how many sessions you might need. Behavioral Health Navigators can provide brief interventions, serve as a mental health coach and remove treatment barriers.
