The Patient Navigator team can help you!

We assist Grand County residents with barriers to healthcare.

  • We coordinate care and work with you to guide you through the healthcare system and to community resources to help you get the care you need when you need it.
  • We coordinate care in cases where there are several different specialists and healthcare providers. This is designed to reduce confusion and frustration for consumers.
  • We educate, clarify, and reinforce diagnosed conditions to help you understand what is going on with you.
  • We assist you with what to do, how to do it, and planning for your best health.
  • We follow-up with you and your healthcare providers to be sure that the plan is on track and you are getting what you need.


PATIENT NAVIGATOR TEAM

Do you need help understanding your chronic disease? Communicating with your doctors?  Understanding your medical bills? Making sure you have everything you need to be healthy? 

Contact our Patient Navigator Team:  

When you need help understanding your disease and medications, how to manage your disease, and how to talk to your doctor(s). These Nurse Navigators can teach you tools to stay healthy and plan for your best health. They can also helps coordinate care between different specialists and other doctors, plus link you to other community resources.  Our Navigators help guide you through the confusing healthcare system so you can get the care you need when you need it.

Are you on Medicaid or Medicare and need resources to get and stay healthy?  Contact:

Contact for help getting community resources that help you get care and stay healthy, like gas vouchers, rent or utility assistance, prescription assistance.

Contact if you just need some help getting what you need to get and stay healthy - regardless of insurance coverage.


Barriers to Health Care

Anything that limits you from getting the care you need when you need it. Barriers can include:

  • Transportation
  • Financial situation
  • Health insurance - no coverage or inadequate coverage
  • Insurance co-pays and required deductibles
  • Prescription coverage and affordability
  • Medical bills:  How do I read this? Is it right?
  • Specialist referrals:  Why am I seeing this Dr.? What did the Dr. say? Do I understand it?
